The decade’s biggest structural change in German healthcare — modeled for you
Germany runs the most bed-dense hospital system in the OECD through the deepest reform in its post-war history, while the statutory insurance that funds it slides toward a financing wall. This study turns that upheaval into the assumptions a financial model actually needs.
It covers the acute hospital sector, the GKV statutory-insurance financing frame, and the Pflege (long-term-care) reform as one integrated system — because that is how the money and the demand actually move. The KHVVG reform (in force December 2024) and its March 2026 amendment, the KHAG, replace much of activity-based DRG funding with capacity-based Vorhalteverguetung, deferred to full effect in 2030. If your model still runs on case counts and bed numbers alone, it is about to misstate revenue.
What you get
- A 34-page Word/PDF study covering hospitals, GKV, and Pflege, 2026–2031.
- 12 EFM-branded charts and 9 data tables, each individually sourced.
- Three quantified scenarios (Bear 25 percent, Base 55 percent, Bull 20 percent) with explicit 2031 outcomes for hospital count, GKV hospital spend, contribution rate, and loss-making share.
- A modeler-ready assumptions appendix (Base case plus Bear/Bull flex) covering the contribution-rate path, Vorhalteverguetung share, occupancy, length of stay, wage index, and Pflege demand.
- A source-verification workbook with every figure mapped and checked.
Key findings
- The footprint is shrinking by design. 1,841 hospitals and 472,900 beds in 2024, down from 1,996 and roughly 500,000 in 2013; our Base case sees about 1,600 hospitals by 2031.
- Vorhalteverguetung breaks the volume model. From 2027 (partial) and 2030 (full), roughly 60 percent of operating revenue becomes a capacity-based reserve budget tied to assigned service groups.
- More than half of hospitals lose money. 56 percent ran a deficit in 2024 (RWI); the investment backlog is EUR 30–50 billion.
- GKV is hitting a financing wall. Hospital care is 31.6 percent of GKV spending (EUR 111.4 billion in 2025); the total contribution rate reached 17.5 percent in 2026; the FinanzKommission projects funding gaps toward EUR 40 billion by 2030.
- Long-term care is the secular demand driver and the next funding crisis. 5.7 million people needed care in 2023, rising toward 6.8–7.6 million by 2055.
Workforce is the binding constraint. A nursing shortfall of roughly 280,000 (by 2049) to 500,000 (by 2030).

Methodology
eFinancialModels uses AI-assisted research and drafting tools alongside human research, source verification, and editorial review. Every claim, statistic, source, chart, and recommendation was reviewed and verified before publication. Forward-looking statements are explicitly labeled and reflect analyst judgment. Regulatory instruments are referenced by their title (for example, the KHVVG, the KHAG, and the relevant provisions of Book Five and Book Eleven of the German Social Code).
Pair it with a template
This study supplies the sector context, reform mechanics, and scenario assumptions. To build entity-level cash flows, pair it with one of the healthcare financial model templates at eFinancialModels.com. The study informs the assumptions; the template is the modeling tool.
